New to the District
Students new to a KIBSD school are required to have the following:
Birth certificate
Immunization record – immunizations must be current prior to entry*
SchoolCare Health Enrollment**
Returning Students
Bring any new immunization record to the nurse. Required immunizations must be up to date prior to the first day of class. Update any health and emergency contact information. Talk with the school nurse if you have health-related questions.
Any Student
Immunizations: School Entry Requirements
Prior to school entry, a child must be immunized as required by Alaska state law. All Immunization records must be submitted, up to date, and verified by the school nurse prior to the first day of school. Remember, NO SHOTS, NO SCHOOL!
All students entering or attending the Kodiak Island Borough School District (grades K-12) must be current with the following:
Diphtheria, Tetanus, Pertussis (DTaP/DT/Tdap) Polio (OVP, IPV), Measles, Mumps, Rubella, (MMR), and Hepatitis A and B, and Varicella. (Varicella is required for preschool through grade 6) All children attending the district preschool program are also required to have documentation for the HIB immunization.
Religious Exemption Form
Alaska State Law requires this form to be notarized and renewed each school year after July first (1st) of the current school year to be valid for that school year. If you are completing one of these forms for your child, the completed form must be submitted AT or BEFORE the start of school.
Medical Exemption Form
This form is required to be on file at school and/or childcare when a child is not immunized due to a medical contraindication or immunity. This exemption form only needs to be completed once.

SchoolCare Enrollment
All students need to be enrolled in the digital SchoolCare health and documentation system. KIBSD uses SchoolCare as a tool for nurses to keep parents notified when students visit the health clinic & for parents to provide the school with accurate student medical information. Enrollment is vital for nurses to understand and identify the health needs of students. All information is stored securely and is HIPAA and FERPA compliant. Parents can enroll students by:
Accessing the PowerSchool Parent Portal. Parents will receive an email directly from SchoolCare with a personalized link to complete your child’s record.
Or complete a paper enrollment form provided by the school nurse or secretary.
Tuberculosis Testing
There will be no requirements for KIBSD to assess the TB status of their students for the 2019-2020 School year.
An exception would be for New Immigrants entering the country who have not completed their TB evaluation. Please contact Kodiak Public Health at 486-3319 to make your appointment.
Emergency Care
Please keep your emergency contact numbers up-to-date so that you or another responsible party can be easily contacted. It is important to have home, work, cell and any emergency numbers on file with the school.
In a life-threatening emergency or when it is unsafe for the parent/guardian to transport a student, EMS will be called.
Accidental injuries at school are treated by the school nurse or administration. Parents will be informed and advised if medical care is needed.
If your child is ill, please consider the other students and staff members and keep your child home. Children who become ill at school will have a parent/guardian notified for pick up. Students with a temperature greater than 100.4F will be excused from school and need to be fever-free without medication for 24 hours before returning to school.

If your student has any of the following health concerns, please complete the appropriate forms and return them to the school nurse BEFORE the start of school. Medical Care Plans need approval and signature of a health care provider.

Allergy/Anaphylaxis
Seizure
Diabetes (Daily Insulin)
Diabetes (Insulin Pump)
Medication Administration at School
If medication is to be administered at school, one of the following needs to be complete and on file with the nurse prior to administration of the first dose. All medication must be delivered by a parent or guardian in the original container.
Over-the-counter medication provided by parent/guardian
A short term prescription medication
A long term prescription medication
Screenings
During the school year, the nurses complete growth screenings (height, weight, and BMI). Results of these screenings are entered into SchoolCare. If you do not want to have your child’s growth screening completed you may sign an opt-out form.
Vision and hearing screening are conducted at intervals during a student’s time at KIBSD. Should a concern be identified by the school nurse, you will be notified. Follow-up information from the optometrist, ophthalmologist or audiologist should be reported to the nurse.
Physical Exams
All students participating in KIBSD sports are required to have an annual sports physical. For information about these requirements, please contact the KIBSD Activities Director.
*Prior to school entry a child must be immunized as required by Alaska state law. The requirements currently include the following: diphtheria, pertussis, tetanus, polio, measles, mumps, rubella, varicella, hepatitis A and hepatitis B. Some children may require a specific booster dose during the school year. KIBSD will comply with state law in all matters pertaining to immunization compliance.
**SchoolCare enrollment can be completed digitally through an email link, from the parent portal, or on a paper form. Enrollment is important for nurses to understand and identify health needs of students. Please contact your school nurse with any questions or concerns.
