School Entry Requirements

Entry Age
A child five (5) years of age on or before September 1 may be admitted to kindergarten. A child six (6) years of age on or before September 1 may be admitted to first grade.  The Superintendent is delegated authority to make early-entrance determinations. Students under school age who were previously enrolled in a public school shall be admitted to school at the grade level determined by the Superintendent of designee. (BP 5111/AS 14.03.080)

Birth Certificate
You must provide a certificated copy of your son/daughter’s birth certificate for enrollment.

Immunizations
All students entering or attending the Kodiak Island Borough School District (grades K-12) must be current with the following:
Diphtheria, Tetanus, Pertussis (DTaP/DT/Tdap) Polio (OVP, IPV), Measles, Mumps, Rubella, (MMR), and Hepatitis A and B, and Varicella. (Varicella is required for preschool through grade 6) All children attending the district preschool program are also required to have documentation for the HIB immunization.

All Immunization records must be submitted and verified by the school nurse prior to beginning school. Remember, NO SHOTS, NO SCHOOL!

To meet requirements, students will need MEDICALLY-VERIFIED:
Immunization record of 2 doses of Varicella OR
History of disease recorded on new state required form OR
Valid medical exemption on new state exemption requirements.
Documentation must be signed by an Alaskan-licensed MD, DO, ANP, or PA, stating the child has had the disease.

Tuberculosis Testing
There will be no requirements for KIBSD to assess the TB status of their students for the 2020-20201 School year.

An exception would be for New Immigrants entering the country who have not completed their TB evaluation.

Religious Exemption Form
Alaska State Law requires this form to be notarized and renewed each school year after July first (1st) of the current school year to be valid for that school year. If you are completing one of these forms for your child, the completed form must be submitted AT or BEFORE the start of school.

Medical Exemption Form
This form is required to be on file at school and/or childcare when a child is not immunized due to a medical contraindication or immunity.  This exemption form only needs to be completed once.
