School Entry Requirements: Immunizations
All students entering or attending the Kodiak Island Borough School District (grades K-12) must be current with the following:
Diphtheria, Tetanus, Pertussis (DTaP/DT/Tdap) Polio (OVP, IPV), Measles, Mumps, Rubella, (MMR), and Hepatitis A and B, and Varicella. (Varicella is required for preschool through grade 6) All children attending the district preschool program are also required to have documentation for the HIB immunization.
All Immunization records must be submitted and verified by the school nurse prior to beginning school. Remember, NO SHOTS, NO SCHOOL!
To meet requirements, students will need MEDICALLY-VERIFIED:
- Immunization record of 2 doses of Varicella OR
- History of disease recorded on new state required form OR
- Valid medical exemption on new state exemption requirements.
Documentation must be signed by an Alaskan-licensed MD, DO, ANP, or PA, stating the child has had the disease.

Tuberculosis Testing
Every child new to Kodiak Island Borough School District, or in kindergarten or grade 7 must have a PPD skin test for tuberculosis within 90 days of enrollment.
Your child may be tested at the Kodiak Public Health Department, at your health care provider, or by the school nurse with your written permission.
We recommend kindergarteners and students new to the district complete this at the time of their school physical or when receiving immunizations.
A PPD test is not required if the school is provided with documentation verifying a negative result of a PPD administered within 6 months of enrollment; or a previous positive PPD result.

Physical Exams
All children entering an Alaskan school for the first time are required to have a school physical. Documentation of the completed physical must be provided to the school within 30 days of enrollment. A physical that has been done within a year of enrollment is acceptable.
All students participating in KIBSD sports are required to have an annual sports physical (a school physical is acceptable). This must be done before the child participates in the sports activities and must be repeated every year.
